  • Welcome assigned patients and inquire as to their well-being since their last treatment. Report any complaints or observations to the nurse supervisor.
  • Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
  • Obtain necessary pre and post treatment vital signs and weight.  Report unusual findings to nurse supervisor.

  • Perform administration of Heparin as delegated or as allowed by state law.

  • Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
  • Monitor patients’ response to dialysis therapy and report any unusual findings to nurse supervisor.
  • Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment – document.
  • Discontinue dialysis treatment according to established procedures,
  • Evaluate patient prior to termination of venous access – standing & sitting blood pressure.
  • Obtain Hemostasis and apply appropriate dressings.  
  • Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
  • Perform and record Pre and Post dialysis evaluation, weight and vital signs with initial identification.
  • Responsible for calculating and entry of individual patients’ dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
  • Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits (NVL) 160.
  • Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
  • Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
  • Report any significant information and/or change in patient condition directly to the nurse supervisor. 
  • Observe patient, and conduct machine safety checks according to facility policy.  Report any change or unusual findings to the nurse supervisor.
  • Perform and document any intervention for unusual patient status and document patients’ response to intervention.
  • Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the nurse supervisor.
  • Obtain all prescribed laboratory testing and prepare specimens for collection.

MAINTENANCE/TECHNICAL:

  • Maintain environmental integrity and aesthetics – ensure all areas are safe and clean.
  • Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
  • Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
  • Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
  • Initiate Solution Delivery System (SDS) system. 
  • Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
  • Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.

CLERICAL & ADMINISTRATIVE:

  • Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities. 
  • Enters all treatment data into the designated clinical application accurately and in a timely manner.
  • Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure omitted entries are completed or corrected where appropriate.
  • Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
  • Prepare lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
  • Collect, label, appropriately prepare and store lab samples according required laboratory specifications.

Number of Positions Open: 30 North side of Chicago and 10 South side of Chicago

Purpose: To evaluate and provide training to residents in daily living and social skills in order to assist them in achieving and maintaining their maximum level of functioning and independent community living.

Essential Responsibilities:

  1. Evaluate and provide training to residents in daily living skills including, but not limited to: personal hygiene and grooming (which may involve physical assisting individuals to complete the tasks), housekeeping, laundry and clothing care, meal planning and preparation, money management and budgeting, socialization, and recreation leisure activities.
  2. Assist residents in achieving their maximum level of independent functioning in regards to goals stated in the Independent Habilitation Plan.
  3. Implement group and individual programs which promote growth and development toward independent living skills for residents.
  4. Assist residents in preparation of daily schedules to include performance of household chores, social activities, and other meetings and appointments.
  5. Complete and insure the timeliness of all assigned documents and forms to include daily progress reports, CILA shift reports, van log sheets, incident reports, MAR sheets, etc.
  6. Monitor maintenance of living facility to assure safe, clean and healthy environment for individuals. Perform cleaning and housekeeping duties such as, but not limited to: vacuuming, dusting, sweeping, and mopping, monitor and assist residents with daily bedroom maintenance.
  7. Monitor residents to insure wellness and safety, enforcing facility curfew if required.
  8. Supervise resident’s schedules and activities to insure timely arrival to day program or job site and other appointments.
  9. Supervise and assist residents in taking their medication in accordance with program guidelines.
  10. Attend and participate in all IHP staffing.
  11. Assist in planning and supervising recreational and social program activities.
  12. Plan menus and assist with meal preparation as required. Monitor and observe residents with special dietary needs.
  13. Attend and participate in all staff meetings.
  14. Respond to medical and behavioral emergencies following appropriate Health and Safety Aggression Management, First Aid, CPR, and other procedures as necessary.
  15. Monitor and assist residents with personal grooming and appearance as necessary.

Other Responsibilities:

  1. Transport and accompany residents to appointments and other services, including work centers, as needed.

  3. Check and secure all doors and windows to insure building safety and activate alarm system nightly.

This position is the first and last point of contact with the patient during the patient visit. • Schedule patient visits using established clinic protocols • Enter patient demographics into practice management system • Run eligibility request to obtain insurance benefits for every patient • Greet patient as they are checking in and checking out • Communicate and coordinate financial arrangements with patients for collection of co-pays deductibles and outstanding balances prior to the patient appointment • Collect co-pays and outstanding balances at point of service and entry into practice management system • Explain to patients our notice of privacy practice, payment policy and consent for treatment and have them sign those documents • Upload to EMR scanned signed copies of those forms along with any former medical records, intake forms, insurance card(s), patient identification and any other documents • Have patients sign a lien for third party car accidents, liability claims or personal injury cases • Ensure receipt and entry of referral source information into practice management system for therapy services • Check if pre-authorizations or pre-certifications are needed for therapy services and obtain them as needed • Communicate with referring providers to receive signed plans of care for OT/PT services • Enable portal access for patients • Weigh patients and record vitals in EMR • Draw blood, print labels and prepare samples for lab pickup • Enter medical records requests into medical records request log • Route phone calls throughout the office • Electronic messaging to staff members regarding patient calls etc. • Route answering service or phone messages from after our calls • Charge patients for any supplements and record sales to update inventory list • Balance co-pays and other monies taken over-the-counter at end of day • Ensure that every visit was charged for by the rendering provider • Rout fax documents and mail to proper staff members • Maintain appearance and cleanliness of waiting room • Ultimate responsibility to ensure that all correspondence is filed in electronic medical records • Upload future appointments to patient reminder calls software • Other duties and special projects as assigned

GENERAL SUMMARY: Provides care and oversight of patients in the prep areas of the Diagnostic Services Department under the auspices of the Radiologist. Actively interacts with patients and family members to assure they are properly prepared for their imaging exam and any issues or needs are addressed. Knowledgeable and proficient in the performance and processing of ECGs and provides support of the ECG Department on an as needed or scheduled basis.   • Provides flexibility in work schedule. Must be able to assume assigned shift between 6am – 2am 7 days/week.
  • Provides basic care needs of the patient while in Diagnostic Services Department in support of the Nurse or Tech.
  • Basic competency in performance and processing of ECGs. Performs ECGs in Diagnostic Holding Area when needed. Available for assignment to ECG Department staffing rotations.

  • Responsible for maintaining a safe and orderly environment and for the wellbeing and safety of the patients under their care as well as that of family members and/or visitors within the examination modality area.
  • Report any incidents, accidents, or inconveniences involving patients to the Tech/Nurse on duty as well as the Department and be prepared to file a full and detailed incident report as needed.
  • Monitor patients for contrast reaction, distress and hypoglycemia; notify Radiologist with any concerns. Initiates Code Blue as per hospital policy when necessary.
  • Escort patients, as needed, to and from various modalities and inpatient units in order to ensure patient

Qualifications:

  • Prior formal clinical training is required. This may include CMA, CNA, PCT, EMT, corpsman, paramedic, and nursing or medical student experience.
  • Excellent written and verbal communication skills and good typing skills.
  • Strong organizational skills and the ability to multi-task.
  • Must demonstrate excellent customer service skills and be able to maintain a high degree of professionalism while working in a high-volume, high-stress environment.

Position Summary: Utilizes a defined skill set consistent with the basic educational program, certification, and relevant work experience to deliver direct care to the patient in the ambulatory setting. Working under the direct supervision/licensure of the physician, the Medical Assistant is an integral member of the health care team and is responsible for coordinating the pre/intra/post aspects of the medical visit. Those include direct care, registration & scheduling, physician-patient communication and other office-related tasks. In addition, will administer medication and works directly with other health care professionals at the site as well as ancillary department and services to coordinate care. Position Requirements: Minimum Education: Required: High School Diploma plus training acquired through work experience or education Preferred: Administrative or technical background acquired through completion of 2 – 3 years of college Minimum Experience: Required: None Preferred: 1-2 years of previous job-related experience Minimum Experience Details: Ambulatory experience in primary or specialty setting is preferred. Licensure/Certifications: Required: CPR (Cardiopulmonary Resuscitation), Certified Medical Assistant (CMA) – American Association of Medical Assistants (AAMA) OR Registered Medical Assistant (RMA) – American Medical Technologists (AMT)

Position Responsibilities: Direct Care: Provides care to the patient and support to the physician throughout the entire visit, beginning at the time of arrival at the reception desk, during the visit, and completion of the checkout process. Clinical Operations: Uses planning and collaboration skills to maintain optimum work flow in the clinical area, including flexing assignments, cross coverage and continuous communication with peers to identify obstacles. Registration & Front office: Utilizes technical skills and knowledge of the computerized scheduling systems to generate patient demographic info as well as facilitate appointment scheduling and access to care. Scheduling: Demonstrates initial and ongoing competency with the enterprise’s computerized scheduling systems for clinic appointments and ancillary testing. Telephone & Communications: Serves as the primary source of contact for clinical operations within the department.
